Test Information
In screening for early acute MI, recommended protocol is to obtain a baseline (zero-hour) sample plus one additional sample 2-4 hours later. A doubling of the zero-hour sample or a 40 ng/mL increase over 2 hours is suggestive of acute MI. Two normal myoglobin results along with a negative TnI or CK-MB in the baseline sample effectively rules out AMI. Myoglobin may disappear from serum within 12 hours after symptoms; accordingly an additional marker such as Troponin I or CK-MB should always be obtained on the baseline sample if the time of onset of symptoms is not known.
