Test Information
Notify the Transfusion Service IMMEDIATELY after a transfusion reaction or suspected transfusion reaction occurs. Transfusion Service personnel will perform the required laboratory tests.
Nurse is required to complete the Investigation of Reported Transfusion Reaction form #25081519​
