Collection Processing Instructions

Marshfield Center Lab: Plasma must be separated from red cells within 7 hours of collection. If assay cannot not be performed within 7 hours, plasma may be stored up to 24 hours refrigerated, or at -20 degrees Celsius for longer storage. Preferred storage is frozen. For add-on (U-have) testing, EDTA whole blood samples (pink or lavender top tube) stored up to 7 hours at 2-25 degrees C may be used to obtain the plasma sample.

Regional Center Labs: Whole blood must be analyzed within 7 hours of collection. If assay cannot not be performed within 7 hours of collection, then centrifuge and remove plasma within 7 hours of collection, and send to Marshfield Center lab for testing. Plasma may be stored up to 24 hours refrigerated, or at -20 degrees Celsius for longer storage. Preferred storage is frozen. For add-on (U-have) testing, EDTA whole blood samples (pink or lavender top tube) stored up to 7 hours at 2-25 degrees C may be used to obtain the plasma sample.

Interference
​Hemolyzed specimens should be avoided whenever possible. However, hemoglobin (up to 500 mg/dL), triglycerides (triolein up to 3000 mg/dL), bilirubin (up to 20 mg/dL) or human serum albumin (up to 1500 mg/L) do not interfere with the recovery of BNP per Biosite literature.

​Normal values <100 pg/mL. The decision threshold of 100 pg/mL translates into a general specificity greater than 95%, i.e. less than 5% expected false positives in individuals without Congestive Heart Failure (CHF).
BNP levels in CHF:
Less than 200: Likely compensated
200-400: Moderate CHF
Greater than 400: Severe CHF.
 
Normal values <80 pg/mL:  The decision threshold for predicting survival and future heart failure in patients with acute coronary syndromes is 80 pg/mL.
The decision threshold for determining heart failure in apparently healthy individuals is 100 pg/mL.  This BNP value translates into a general specificity of the test of greater than 97%.
Less than 200:  Likely compensated
200-400:  Moderate CHF
Greater than 400:  Severe CHF
