25508 Protein-Immunotyping (PEP,GAM,IT) Panel

Protein-Immunotyping (PEP,GAM,IT) Panel
Test Code: IEPIT
Synonyms/Keywords
IF Panel (if Immunotyping is required by interpreter), IEP, Immunoelectrophoresis panel - includes protein electrophoresis, immunoglobulins IgG, IgA and IgM and Immunotyping if needed)
Test Components
Protein Electrophoresis (Total Protein, Albumin, Alpha-1, Alpha-2, Beta, and Gamma Globulins), Immunoglobulin IgG, IgA and IgM.  Immunotyping, if required by interpreter (there is an additional charge for Immunotyping).
Useful For
Protein electrophoresis is a useful tool in screening or monitoring patients with protein abnormalities such as hypoproteinemia, hepatodegenerative disease, acute and chronic phase reactions, anemia, polyclonal and monoclonal gammopathies and dysproteinemias.  Quantitative immunoglobulins are useful in detecting or monitoring of monoclonal gammopathies or immune deficiencies.  Immunotyping is useful in determining or ruling out a monoclonal gammopathy.

If Immunotyping is required - an additional 24 hours should be expected

Protein electrophoresis - agarose gel/Sebia Capillarys 2 Protein System

Immunoglobulins – Turbidimetric method / The Binding Site, Optilite analyzer

Immunotyping electrophoresis/Sebia Capillarys 2 Protein System

Test Information
Includes serum protein electrophoresis by Capillary Electrophoresis and quantitative immunoglobulins (IgG, IgA and IgM). Phenotyping of heavy chains and light chains (kappa and lambda) may also be performed by Immunotyping if required for pathologist/PhD interpretation and will be charged separately. If the ordering physician wishes to obtain Immunotyping electrophoresis (IF) regardless of the serum protein pattern and quantitative immunoglobulin findings, add comment to order: "Do IF regardless". However, if a monoclonal gammopathy has been established and the M-spike not appreciably changed since the last SPEP, the pathologist/clinical chemist may elect to enter a comment that states "Requested immunofixation procedure not performed as M-protein specifically for the patient has already been established."

Total Protein:

0-12 months: 4.0-7.8 g/dL

13 mo.-9 yr: 6.0-8.0 g/dL

10 yr.-19 yr: 6.4-8.6 g/dL

 20 yr. and older: 6.0-8.0 g/dL

Albumin:  3.70-4.90 g/dL

Alpha-1 globulin: 0.20-0.40 g/dL

 Alpha-2 globulin: 0.50-0.90 g/dL

Beta-globulin: 0.60- 1.00 g/dL

Gamma-globulin: 0.60-1.40 g/dL

Interpretive report included.

Note: Occasionally due to round-off error, the sum of all fractions may not exactly equal Total Protein.

 Immunoglobulin Reference Ranges

Interpretation
Increased serum immunoglobulin concentrations occur due to polyclonal or oligoclonal immunoglobulin proliferation in hepatic disease (hepatitis, liver cirrhosis), connective tissue diseases, acute and chronic infections, as well as in the cord blood of neonates with intra uterine and perinatal infections. Elevations of IgG, IgA, or IgM may occur in monoclonal gammopathies such as multiple myeloma (IgG, IgA), macroglobulinemia (IgM), primary systemic amyloidosis, monoclonal gammopathy of undetermined significance (MGUS), and related disorders.
Decreased levels are found in patients with primary or secondary immune deficiencies.
